Thomas Richard Forshaw was born in Toxteth, Liverpool on January 3rd 1886. His father, Thomas P. Forshaw, was a Cocoa Rooms manager.

Thomas spent all the early years of his life in Liverpool. He became a Cook and was employed as a Chef.
He married Violet H. Wooley of Barden Road, Speldhurst, Tunbridge Wells, Kent.

In Dec 1914, he enlisted in the King’s Liverpool Regiment. After training he joined his Battalion in France and was present at the Battle of Loos.

He died at an unspecified 'Rainhill Military Hospital (Lancashire)' on January 10th 1918 aged 31.
His grave is in Toxteth Park Cemetery, Liverpool
